Topanga Terrace Rehabilitation and Subacute is a 112 bed facility with a 5-star overall Quality of Care rating from the Center for Medicare and Medicaid Services. We offer a warm, nurturing environment for our patients, and specialize in rehabilitation, subacute, and skilled nursing care.

Our facility services the highest acuity patient care needs in the West Valley after a hospital discharge. We offer specialty programs for respiratory therapy, stroke rehab, and orthopedic surgery rehabilitation. Our respiratory therapy program services patients with ventilator weaning and tracheostomy decannulation needs, amongst other respiratory care services.

Our highly trained physical, occupational, and speech therapists work closely with experienced physicians, physiatrists, and orthopedic surgeons to offer an individualized and integrated plan for each patient. Rehabilitation therapy is provided seven days a week using state-of-the-art equipment to assist our patients with their goal to return home.
